Dr. Daniel Stock calls it the worst missed opportunity of the pandemic, and he is specific about why.
A group of patients already hospitalized with COVID, not yet in the intensive care unit, already on azithromycin and hydroxychloroquine, were randomized to receive either the activated form of vitamin D or a dummy pill. Placebo controlled. Randomized. Double blinded. The trial reduced the risk of going to the intensive care unit by about 90 percent, a result Stock describes as highly statistically significant, and the rate of ICU transfers dropped within days. Deaths fell to zero in the treatment group, though he is careful to say that finding was not statistically significant because the trial was small, roughly 76 patients.
The treatment cost about 30 dollars over the counter. The side effects came in lower than placebo. It was published roughly two months before the first vaccine rolled out.
Stock explains the biology in plain language: ordinary vitamin D is a precursor the liver has to convert, and a liver under stress from serious illness has trouble making that conversion, which is why the already activated form matters when a patient is genuinely sick.
Then he makes his sharpest charge in the hour. When people tell him the agencies did not ignore the vitamin D data, he agrees, and says that is worse. He describes a poorly designed vitamin D trial, wrong dose and wrong measurements, whose researcher he says acknowledged his criticisms, and whose funding he later traced to a group funded by the National Institutes of Health. His conclusion: the response to vitamin D was not silence but propaganda.
