Dr. Brian Tyson was investigated by the California Medical Board for going against the guidelines and protocols. He describes how the conversation ended: two and a half hours in, the investigator looked at him and said he was just doing his job.
That is the setup for the argument that runs through the rest of the segment. Tyson says medicine has to trust the doctors on the front line, and he reaches for a military comparison to explain why. Commanders who do not know what is happening at the front are less effective than commanders who do. What is the enemy doing? What is it using? How do we adjust?
There is no equivalent ground-level channel, he says, at the Centers for Disease Control and Prevention, the Food and Drug Administration, or the National Institutes of Health. Not a single person on the ground telling the administration what is actually happening in medicine, and in his view that is still true today.
He follows it with the clinical reason it matters. What you see in a hospital, he says, is end-stage disease eighty to ninety percent of the time. Judging early treatment by what happens in an intensive care unit gets the answer backwards, and by that standard you could conclude the standard antibiotics do not work either.
His question for the people writing the guidance: if what you are doing is not working, why not try something else?
